Login Name: First Initial + Lastname + Last 4 digits of your SSN

          (Example: JDoe1234)

Password: Full DOB (mmddyyyy) + Last 4 SSN digits + the word "Clinic" -  capital C + exclamation mark "!"

          (Example: 010119851234Clinic!)

Please allow yourself approximately 20 minutes to electronically complete the forms in the Portal.  These forms consist of W-4, Direct Deposit form, etc. It is essential that you complete these forms in order for us to set up your employee data correctly in Ultipro, our Human Resource Information System, by the deadline communicated. Note:  Yellow highlighted areas on the forms are required fields.

The Portal does allow you to save, exit and return at any point should you need to momentarily step away. Remember to save your work prior to exiting, so that you can return to the Portal at any time to complete the remaining forms.
